Student Medical Certificate
Department of Computer Science, Brock University
To expedite the administration of medical certificates, the Computer Science Department requires that this certificate, or the University Health Services Student Medical Certificate, be used by students. When a medical condition requires special consideration for any academic activity (for example, missed seminars, assignment extensions or examination rescheduling), students and their physicians must complete this certificate.
