COSC Database Reference and Student Quotes Questionnaire

 
The COSC Database Reference is used to allow for better consideration for future references from professors, by keeping a visual record combined with achievement. In addition it is useful in seeking student quotes.
 
To be included in the COSC references database, completely fill out the following questionnaire and submit it. Once submitted, have the mentor take your picture to be stored in the reference database along with the rest of the information.
